Inpatient Hematopoietic Stem Cell Transplant Rotation

    Residents will be responsible for providing medication therapy management services to patients receiving autologous and allogeneic stem cell transplants. Patients on this service receive stem cell transplants for both malignant and non-malignant diseases. The residents will participate in daily multidisciplinary patient rounds, manage metabolic support issues, provide pharmacokinetic consults with a focus on immune suppressant agents, pain management consults, drug information, patient / family education, discharge planning and medication reconciliation, and maintenance of allergy and adverse drug event database. Topic discussions on this service will include the front-line Bone Marrow and Cellular Therapy clinical trials, infectious complications and supportive care issues for the stem cell transplant patient. The resident will gain a basic understanding of immunology issues focusing on graft versus host disease and immune reconstitution after stem cell transplant.
