Ambulatory Care Unit – Hematopoietic Stem Cell Transplant Rotation

    Mirroring the inpatient service, the resident will be responsible for medication therapy management services for patients who have undergone hematopoetic stem cell transplant. The resident will be responsible for pharmacokinetic consults for immune suppressant agents, managing home parenteral nutrition, pain management, evaluation of conditioning regimens and development of electronic order sets for patients not treated on a clinical trial, drug information, patient education and medication reconciliation, as well as allergy and adverse drug reaction reporting. The resident will gain pharmacy practice management skills by attending planning meetings for patients prior to transplant, helping ensure that pharmacy services are sufficiently prepared to support the patients during conditioning and their inpatient stay.
