Having an interest in or affiliation with a commercial interest* does not prevent a speaker from making a presentation, but the relationship must be made known to the audience in accordance with the standards of the Accreditation Council for Continuing Medical Education. In order to fulfill this obligation, please assist us by completing the Faculty Disclosure Form regarding relationships held within the past 12 months (this includes spouse/life partner affiliations as well).
Please Note: If this information is not submitted in order to disclose conflict of interest (must be identified and/or resolved) within one week prior to a regularly scheduled series, and two weeks before a course/conference, you will be able to present; however, CME credit will not be granted.
*A commercial interest is any entity producing, marketing, re-selling, or distributing health care goods or services consumed by, or used on, patients.
