Who’s your reason for completing the 2016 Biometric Screenings and Wellness Assessments? Is it your children who depend on you for moral and financial support? Is it you wanting to lead your best life? Or maybe it’s a co-workers who will be inspired by your example.
St. Jude is partnering with Methodist Healthcare to help you understand and manage your overall health and wellness. In April, all St. Jude and GMP employees have an opportunity to take the annual Biometric Screenings and Wellness Assessment. The wellness assessment is an informative tool to help you understand your current health condition. Together, with the results of your biometric screenings, you will have a snapshot of what you’re doing well and areas for improvement as you make important health decisions going forward.

Screenings typically take 15-25 minutes and include:
- finger stick testing for glucose and cholesterol levels (Fasting is not required, but some prefer to fast before having glucose levels tested. If so, we encourage you to choose a morning appointment.)
- weight and height measurement to establish Body Mass Index (BMI)
- blood pressure measurement
After your screening, you will review your results with a screening specialist and have an additional opportunity to discuss healthy living options with a Living Well health coach.
Please note: If you’ve seen your physician since July 2015 you can download the Health Care Provider Form and have your physician complete, sign and fax it to (901) 576-1757. The form must be received by June 3, 2016
Wellness assessments are available through the StayWell portal and must be complete by June 3, 2016. This link will be active soon. The online questionnaire focusses on your health habits and is equally important to give you a total picture of your health. Assessments typically take 15-20 minutes to complete. Login is required; call 1-855-719-1905 for help with logging in.
Everyone who completes the Biometric Screening will be entered into a drawing for a chance to win one of five FitBit wellness tracking devices!
Live Well. Earn Rewards. incentive program
The screening and assessment are at the heart of Living Well’s Live Well. Earn Rewards. incentive program. Completion of both are required to be eligible for rewards in the incentive program.
Along with the biometric screenings and wellness assessment, St. Jude and GMP employees have opportunities to participate in activities throughout the year and accumulate points that can be redeemed for prizes at the end of the fiscal year.
Points earned at each of these levels can be redeemed for fabulous prizes, including jewelry, outdoor gear, wellness & sports equipment, luxury cooking appliances and much more!

In June, those eligible for prizes will receive an email with a link to the online rewards catalog.
Your privacy is important to us. Third-party vendors are conducting the screenings and St. Jude is only provided aggregate information to help us understand the overall health of our employee population and where our greatest wellness emphasis should be. Your personal health information, medical conditions and health results are kept strictly confidential. The Living Well health coaches, who are not St. Jude employees, can assist with lifestyle behavior changes that are specific to the needs identified through your personal wellness assessment and biometric screening results.
