Receive Tax Document by Mail
Complete the form below to get your annual donation statement by mail. Once requested, statements are mailed out annually starting in February. Note: you will only receive your statement for this tax season if you submit the form between January and August. If you submit the form between September and December you will only receive your statement by mail for the following tax year. You only need to submit this form once, after that, you will continue to be mailed your statement annually.
