I give the St. Jude Children’s Research Hospital (St. Jude) and the American Lebanese Syrian Associated Charities (ALSAC), the exclusive fund-raising organization for St. Jude, permission to use my name, likeness, image, voice, and/or appearance as such may be embodied in any pictures, photos, video recordings, audiotapes, digital images, website and the like, taken or made on behalf of all St. Jude Science Scholars of Tomorrow activities.
- I agree that St. Jude and ALSAC will have complete ownership of the pictures, including the entire copyright, and may use them for any purpose consistent with St. Jude’s mission. These uses include, but are not limited to illustrations, bulletins, exhibitions, videotapes, reprints, reproductions, publications, advertisements, and any promotional, fundraising, or educational materials in any medium now known or later developed, including the Internet.
- I acknowledge that I will not receive any compensation, for the use of the pictures, and I waive any right that I may have to inspect or approve the finished product or products and the copy or other matter that may be used in connection with them or the use to which they may be applied.
- I specifically consent to the digital compositing or distortion of the portraits or pictures, including without restriction any changes or alterations as to color, size, shape, perspective, context, foreground or background. I also consent to the use of any published matter in conjunction with the photographs or other media.
- These materials may be taken by and for St. Jude and ALSAC and their representatives and person(s) authorized by them. The purposes of these groups may be to share my image with the public, including but not limited to, newspapers; magazines; television; radio; video projects; comments or postings on the Internet, blogs and social media websites such as YouTube, Facebook, MySpace, Twitter; oral testimonials in public venues, and publication in other media by celebrities and supporters.
- I release, discharge, and agree to hold harmless St. Jude and ALSAC and their agents and assigns from liability arising from publication of my likeness, including without limitation claims for libel or violation of right of publicity or privacy.
- I have read and understood the above authorization, release, and agreement prior to its execution, and I am fully familiar with the contents of this document. This release and consent shall be binding upon me and my heirs, legal representatives, and assigns.
- I give my consent to St. Jude and ALSAC to use my name and likeness to promote any St. Jude Science Scholars of Tomorrow, St. Jude, and ALSAC program and/or activities.
