Living Well Pharmacy
Living Well Pharmacy is an onsite full service pharmacy for employees and dependents enrolled in the St. Jude medical plan. Experienced pharmacists who go beyond just prescription fills/refills. They will tailor a care plan to your unique needs, help you with medication adherence, while taking the time to discuss your medications and overall health concerns.

Your personal health information is considered private and confidential. St. Jude has hired Premise Health, who maintains a separate system for all information related to personal medical visits. Premise Health complies with all provisions of the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and subsequent amendments.
ELIGIBILITY
Living Well Health & Wellness Center: St. Jude, GMP and Graduate School employees that are also enrolled in St. Jude’s medical benefit plan.
Living Well Pharmacy: Employees (who fit the same criteria as the above), and their dependents can fill prescriptions at the pharmacy. Only employees will be able to pick-up the prescriptions filled at the Living Well Pharmacy for themselves or their dependents.

No, the Living Well Health & Wellness Center is staffed and powered by Premise Health. Premise Health manages the Living Well Health & Wellness Center in the same manner as any licensed medical office in the community.

All preventive services are covered at no cost. Copays for all other services at the Living Well Health & Wellness Center is $10 (unless the visit falls under a wellness/preventive service) as opposed to $15 in the community.

You can request that your PCP share your records with the Living Well Health & Wellness Center by completing your provider’s record sharing process. Additionally, you may request your records from Premise Health be shared with your PCP or specialist.

Premise Health has recruited a high-quality team of comprehensive, holistic health and wellness professionals to staff and manage the Living Well Health & Wellness Center. The team includes a Physician, Nurse Practitioner, Licensed Practical Nurses, Condition Management Registered Nurse, Behavioral Health Specialist, and Pharmacists. Also available are Living Well resources such as an Mental and Emotional Support Services powered by Resource for Living, BCBST Consultant, TIAA Consultant, Living Well Fitness Staff, and a Change Condition/ Wellness Coach that can help manage chronic condition(s).

The Living Well Health & Wellness Center operates in accordance with HIPAA and works diligently to protect all health records. Premise Health cannot share any personal health information without your permission. Your personal health information is not shared with St. Jude. Notice of privacy practices will be available for review within the Living Well Health & Wellness Center.
