St. Jude MyChart is a secure, internet-based way to access and share some of your confidential medical information.
In this document, we refer to St. Jude Children’s Research Hospital, Inc. as St. Jude, we, us, and our. We offer St. Jude MyChart to our patients and authorized parents or legal guardians (you, your), subject to these Terms and Conditions of Use (Agreement).
By clicking “ACCEPT” below, you agree that you:
- Have read this Agreement
- Understand this Agreement
- Will be bound by this Agreement
If you do not agree to be bound by this Agreement, click “DECLINE.” If you decline, you will not be able to use St. Jude MyChart.
Never use St. Jude MyChart for medical emergencies or urgent situations. For any urgent medical needs, call your doctor or 911 right away or go to an Emergency Room. St. Jude does not have an Emergency Room.
We may suspend or end your access to any or all St. Jude MyChart account features at any time for any reason, including:
- If you do not follow this Agreement
- If you abuse your access privileges
Examples of abuse:
- Using offensive language toward your care team
- Making threats toward any St. Jude staff
Notice of Health Information Privacy Practices
Your privacy and the security of your health information is important to us. We strive to keep your health information confidential. Learn more about our privacy practices at the St. Jude Notice of Privacy Practices (HIPAA) webpage.
Access to St. Jude MyChart
Below are the types of access to the patient’s medical record through St. Jude MyChart.
Minor Access
Patients ages 14–17 may access their medical information by creating their own St. Jude MyChart account. We encourage teens to take part in their medical care. They may use St. Jude MyChart to:
- Schedule appointments
- Ask questions
- Get information
Legal Consenter Access
Parents or legal guardians (Legal Consenters) of patients younger than 18 can request access to the patient’s medical information by asking for a Legal Consenter account. We can give this access only to people with:
- Parental rights
- Legal custody
- Guardianship
- Power of attorney or similar, or
- Other rights to access the patient's medical information
If you can have access, we will activate a separate Legal Consenter St. Jude MyChart account for you. Your legal relationship with the patient might change and affect your authority to access the patient’s medical record. If this happens, you or the patient must let us know right away. Under the law, some patients may be able to limit the Legal Consenter’s access to some types of medical information.
Adult Patient Access
In most cases, St. Jude ends a Legal Consenter’s account when the patient turns 18. Most patients can manage their own accounts at this age. The patient may allow another person to access their medical information in a separate account.
Non-Consenter Access
Sometimes people other than the Legal Consenter may get their own accounts to see part of the patient’s information. These people might include family members, emergency contacts, or caregivers. The Legal Consenter must approve these extra accounts if the patient does not have Adult Patient Access.
Our St. Jude Access Only
Sometimes family members or caregivers may need access that does not include patient treatment details. This access is limited to things like:
- Clinical appointments
- Travel plans
- Housing reservations
When the Legal Consenter approves, an individual may be given access to these details through a mobile app called Our St. Jude.
Rules for using St. Jude MyChart

You are responsible for remembering and safeguarding your St. Jude MyChart username, password, and security question and answer.
It is important to keep this information confidential. Anyone with your St. Jude MyChart username and password could look at your medical information. That person could also use St. Jude MyChart as if they were you. This includes reading your messages and sending messages as if they were you. Change your password right away if you are concerned about the security of your username and password. You may change your password at any time. You agree not to hold St. Jude liable for any damages if you do not keep your username and password secure.

St. Jude MyChart might not work from time to time. This could happen because of system failures, back-up procedures, maintenance, or other causes. If this happens, you can reach out to the St. Jude staff and access your health information in one of these ways:

You may deactivate your St. Jude MyChart at any time. Call the St. Jude MyChart staff at (901) 595-4636.

Once we activate your St. Jude MyChart account, you can access health information in it. St. Jude MyChart will send email to the address you give us. You may get emails such as:
- Notice that new lab or test results are available to view in St. Jude MyChart
- Notice that a new Message is available to view in St. Jude MyChart
These emails will not include confidential health information. Any person with access to your email address will be able to see all notices.
You must verify and update your email address through the My Preferences link. This will ensure you get information on time. St. Jude is not responsible for any damage if you do not do this.

We monitor our network and computer systems for lawful purposes. We aim to protect security and to prevent or stop unauthorized access. You agree:
- To not interfere or try to interfere with the proper working of St. Jude MyChart or any activity conducted on it
- To not post or send any unlawful, threatening, fraudulent, libelous, defamatory, harassing, or offensive materials in St. Jude MyChart
- To not use St. Jude MyChart for any illegal or unethical activities
- That hacking or other efforts to access unauthorized areas or information through the website hosting St. Jude MyChart may lead to St. Jude terminating your account and potential legal prosecution.
St. Jude MyChart staff may investigate violations. They may also cooperate with law enforcement to prosecute users involved in such violations.

You may receive messages, test results, and other communications in St. Jude MyChart. You might not see all of your medical record on St. Jude MyChart. St. Jude puts medical information in St. Jude MyChart based on our policies and applicable laws.
By using St. Jude MyChart and accepting this Agreement, you will receive electronic access to the patient’s medical information. This means you might not get certain documents in paper form unless you ask for them. You may ask for paper copies in one of these ways:

- Visit the St. Jude Health Information Management Department in the PCC Building, Room BP270.

St. Jude MyChart has a message center you can use to send messages to your St. Jude health care team. Communications using the St. Jude MyChart Message Center are sent over a secured, encrypted connection. So, we strongly recommend you use St. Jude MyChart Message Center instead of email. To respond to your message, your health care provider might share your message with other authorized St. Jude employees. The staff might place some messages you send through the message center into your medical record. We will respond to your St. Jude MyChart messages as soon as we can during our standard business hours. We do not guarantee response time.

St. Jude MyChart has features that allow you to share parts of your health information or transmit it to other health care providers who are not a part of St. Jude. You must decide who you will share this information with.
You are responsible for the use of these features. Be aware that the person who receives the information could use or disclose it after it is transmitted or downloaded. If you share parts of your health information, St. Jude is not liable for any damages based on those actions.
You agree to hold St. Jude harmless of any damages resulting from such a release. You waive all St. Jude liability related to such a release. You agree St. Jude cannot and does not verify the designated recipient received your record.

When you access St. Jude MyChart, information will be sent over the method you choose to connect with St Jude MyChart on the internet. Your selection of an internet connection method is beyond our control. We are not responsible for your choice of connection method.
You assume the sole risk of any: Unauthorized Access, Disclosure, Intentional intrusion, Delay, Failure, Interruption, Corruption of data or other information when connecting with St. Jude MyChart.
We use industry-standard encryption technologies to protect and secure St. Jude MyChart accounts. This includes the secure messaging function. But, no system can perfectly guard against intentional intrusion or accidental release of information. St. Jude MyChart is not error-free. You will hold us harmless for any errors. Once St. Jude receives the information, we will treat your medical information as confidential. We will give it the same protection we give other St. Jude medical records.

We strive for integrity and accuracy in St. Jude MyChart. But we make no representations, warranties, or guarantees about the accuracy of any St. Jude MyChart content.
It is possible the information on St. Jude MyChart could include typos, incorrect facts, or other errors. It is also possible that third parties could make unauthorized additions, deletions, and changes to St. Jude MyChart. If you see something that is not accurate, please contact us in one of these ways:

You assume the sole liability for your actions in using St. Jude MyChart. You will hold us harmless and defend us against all damages related to:
- Your use of St. Jude My Chart or
- Directions that you give to us and that we rely on.

St. Jude may at any time and without notice to you:

- Suspend or end your use of St. Jude My Chart

We will not be liable to you for any such action.

St. Jude reserves the right at any time to modify, alter, or update this Agreement with or without notice. Changes become effective as soon as they are posted on St. Jude MyChart. You should visit this page from time to time to review the current Agreement. Your continued use of St. Jude MyChart after we post changes means you agree with the modified Agreement.

If a court or other tribunal finds any term or part of this Agreement unenforceable, that term or part shall be limited or eliminated to the smallest extent needed so this Agreement can remain in full force and effect.
This is the entire Agreement between St. Jude and you for St. Jude MyChart. If you have a dispute or claim relating to St. Jude MyChart or this Agreement, you agree to resolve it in the state or federal courts located in Shelby County, Tennessee, following Tennessee law, without regard to its choice of law provisions. You waive all objections to jurisdiction, venue, or inconvenient forum.
