Welcome to St. Jude

As you move forward with your child’s care, we at St. Jude Children’s Research Hospital want you to know that you are not alone. We pledge to work with you to advance your child’s health, happiness, and well-being. Your family is an essential part of the care team. We want to help you become an active partner in your child’s care, both at St. Jude and with other care providers in your local community. 

St. Jude offers many education materials to help you understand your child’s care. All of these materials can be helpful, but please know that our staff members are your greatest resource.   

We are always happy to answer your questions and help you. We want you to feel that you are part of the St. Jude family.

Documents to bring with you

For your first visit to St. Jude, we need certain information to set up your medical chart correctly. Be sure to have the following information with you:

• Child’s birth certificate
• Child’s Social Security number (if a U.S. citizen)
• Your Social Security number (if a U.S. citizen)
• Copies of custody or divorce papers (including court orders and parenting plans), guardianship papers, and power of attorney documents to show who can sign for your child’s medical tests and treatments
• Photo identification (driver’s license, ID card)
• Insurance card(s)
• Employer information for the patient, parent(s), or guardian(s) 
• Emergency contact(s) name, address, and phone numbers
• Insurance information (including medical, pharmacy, and dental insurance cards)
• Family doctor’s name, address, phone, fax, and email address
• Name of the doctor your insurance company assigned as your child’s primary care physician (including address, phone, fax, and email address)

Clinic
Monday–Friday: 7 a.m.–5 p.m. 

If your child needs medical care when the clinic is closed, check in at Patient Registration. If you arrive after 10 p.m., you will be greeted at the front desk by a security guard who will call staff in the Infusion Center to let them know you have arrived.

Call the Infusion Center if your clinic is closed. Please call before coming to the hospital.  

If your child has a fever or other signs of an infection 

Call your child’s primary clinic right away.  

If there is an emergency while at the hospital

Notify your nearest care team member, and then dial:  

• Ext. 4444 (from any hospital phone) 
• 901-595-4444 (from a cell phone) 

If there is an emergency outside of the hospital buildings

Call 911. 

If your child might have an illness that could spread to others 

Call 901-595-3300 before coming to the hospital. Ask for the nursing supervisor and let them know that your child may be ill.
