Occupational Health Requirements

Your parent adviser position requires that you complete additional occupational health requirements. This is determined by the frequency of your visits to campus and the level of interaction between you and other caregivers and patients in active treatment. We appreciate your help in keeping our patients' safety as our top priority.

There are two ways you can complete these additional steps.

  • Complete the Immunization History Form by taking it to your healthcare provider or walk-in care clinic. Return the completed form via fax to 901-595-2720 or email to volunteers@stjude.org.
  • Alternatively, you may make a "new volunteer" appointment with our Occupational Health office by calling 901-595-2531. Appointments are available Tuesday, Wednesday and Friday between 7:30 a.m. and 4 p.m. You should plan for a second visit within 48 hours if you require a TB skin test. Please note, if it has been longer than two weeks since your visit to Occupational Health, and you have not received clearance from the Volunteer Office, contact us at 901-595-3328 so we may follow up.

We highly recommend completing the form on your own. By doing so, you will:

  • Save St. Jude significant resources
  • Expedite your onboarding process, allowing you to volunteer quickly

If you need any help with this process or have additional questions, please contact us at PFCC@stjude.org.  Once we’ve received your completed health immunization form or clearance from the Occupational Health nurses, we’ll be in touch to schedule the training for your specific parent adviser position.
